Requirements for admissions

• Must have a Doctor’s note approving the enrollment into the day care
• Must be free of communicable diseases
• Must meet the criteria to be accepted into facility:

    Participants that shall be allowed into the facility program:

• Independent feeders
• w/c and walker users
• incontinent and continent persons
• persons with feeding tubes
• persons with medications that need to be monitored
• diabetics
• persons with oxygen
• persons with dressing that needs to be changed/cared for
• persons that need help with adl’s
• Alzheimer’s/Dementia patients
• Persons that need physical therapy

Participants that shall not be permitted into the facility:

• Persons that have extensive medical needs
  • Machines that I would have to purchase
  • Severs disabled persons
• Persons needing lifts to move
• Persons that are bedridden
